National Shared Care Plan website live

Information for health providers and patients about the National Shared Care Plan Programme has been made available online at www.sharedcareplan.co.nz.  

The National Shared Care Plan programme has been developed in partnership with the National Health IT Board and the Auckland Regional District Health Boards. The programme comprises three pilot projects – one each for Auckland, Counties Manukau and Waitemata DHB’s, working with respective Primary Health Organisations and Primary Health Care Practices and selected hospital based services. 

The pilot projects are managed by healthAlliance, using HSAGlobal's care management solutions.
