HSAGlobal Singapore NewsConcordia CCMS was successfully piloted for integrated care management in SingHealth and has been announced in the local media. The pilot involved caring for patients in the ward, specialist outpatient clinics, polyclinics and community settings. Concordia CCMS enabled care providers to refer care electronically downstream and upstream within the healthcare continuum, providing visibility on handoff of care, measurement and results, clinical documentation and careplan for care providers to monitor patient’s progress and modify care accordingly. Local NewsThe Singapore government announced that for FY’09, the Ministry of Health is investing $200 million to develop an electronic health records system that is accessible by authorised medical practitioners at the hospitals and polyclinics, and eventually extending to the community care sector. It will allow for more effective treatment of patients who may receive a spectrum of healthcare services from different providers, making Singapore among the first in the world to implement such a system nation-wide. |
