CCMS interRAI Home Care (HC)

interRAI is a collaborative network of researchers in over 30 countries committed to improving health care for persons who are elderly, frail, or disabled. interRAI has developed a range of instruments and screeners to support assessment in a wide array of community and institutional settings. interRAI protocols approach an assessment from a “whole of person” perspective, integrating elements such as mobility and self-care into a comprehensive assessment tool. Each instrument in the interRAI family of tools and applications has been developed for a particular population, and they are designed to work together to contribute to an integrated and consistent healthcare delivery system.
 

HSAGlobal has developed a software solution to automate use of the interRAI HC instrument, enabling users to generate interRAI data, assessments and care plans quickly with reduced manual input. Also, the solution allows users to integrate the interRAI outputs into their existing clinical systems, including HSAGlobal’s Collaborative Care Management Solution (CCMS) platform, or use the software as a stand-alone mobile application.
